If the hammer spring gets weak, or if the firing pin hole gets washed out that can happen. Simple enough to cock the hammer and drop it and then open the breech block, the primer should be set back into place. But getting a gunsmith to look at the firing pin, and hole in the breechblock is the next step in a permanent fix.
